Registration for your outpatient procedure or overnight stay, including surgery or delivery of your baby, can be done when you arrive at the hospital or in advance of your arrival.

To assist with the admission process, we will ask you for certain information and documents, such as:

  • Social Security Number
  • Medicare card, if applicable
  • Military ID card, if applicable
  • Insurance card(s)
  • Insurance claim forms (completed and signed), if applicable
  • Physician referral/authorization forms
  • Co-pay, deductible or co-insurance
  • Copies of your advance care planning, if desired

One of the most important conversations you can have with your health care providers and your loved ones is about what kind of care you want if you should become critically ill or are unable to speak for yourself.

BILL PAYMENT & FINANCIAL INFORMATION

We will submit a bill for your care to your health insurance provider on your behalf. Any deductible, co-payment and co-insurance amounts that are indicated by your insurance provider as your responsibility are requested at the time of service.

Remember, you will receive a separate statement from each physician involved in your care. Charges for services provided by your surgeon, consulting physician, anesthesiologist, pathologist, emergency physician or radiologist are not included as part of your hospital bill.

FINANCIAL ASSISTANCE

We know that paying for medical care can be difficult, particularly for patients who lack health insurance. Based upon eligibility, a patient may receive a discount on all or some of the facility charges related to the care received. Payment arrangements may also be made.
