National Case Management Week 2018 is October 7-13
This year’s American Case Management Association theme, “Case Management: Improving Outcomes. Improving Care” was selected to emphasize the impact professionals in case management make every day.
“This is the Case Management motto,” says Capital Medical Center Case Management Director Sonji Lightbourn. “We strive to ensure each patient is seen by Case Management in a timely manner to ensure patients’ needs are met.
One area Lightbourn’s team strives for each day is a decrease re-admissions by fully assessing the patient needs prior to hospital discharge – something the team does every day of the week to assist nursing and physicians with discharge planning.
Lightbourn adds her department team members take pride in helping every patient and their family members.
“We are here to assist them and their loved ones,” Lightbourn says. “Our goal is to help patients and family members meet their goals for a safe discharge. We work with helping patients with Rehab placement into Skilled Nursing Facilities. We work with patients and families to find assisted Living and Adult family homes. We also work with families to help their love ones transition to hospice care.
“We are a small but effective team and helping is our goal,” she adds. “We have very knowledgeable nurse case managers as well, and awesome social workers. When we work together with families, we have good outcomes. We care!”
